This notice is published in terms of the Promotion of Access to Information Act 2 of 2000 ("PAIA"), which gives effect to your constitutional right of access to information held by the Practice.

The Practice holds patient medical records, appointment and booking records, and standard business/financial records. Patient records are held in confidence and are only accessible to authorised clinical and administrative staff.
To request access to information held by the Practice (including your own patient records), please submit a written request to the Information Officer using the contact details above, specifying the information requested and the reason for the request. Requests for access to your own medical records will be handled in line with applicable health record regulations and professional guidance.
Reasonable fees may apply for the reproduction of records, in line with the fee structure prescribed under PAIA.
If you are not satisfied with the response to your request, you may lodge a complaint with the Information Regulator of South Africa at inforegulator.org.za.
